Scheduling at Embody PT
To schedule a new evaluation please contact Elayne by text/phone at 802-560-8757 or email at elayne@embodyptvt.com. If you are a returning patient, you can schedule a treatment session 3 ways - online (below), by calling or texting 802-560-8757 or emailing elayne@embodyptvt.com. It is typically easier to get ahold of me through text or email as I'm often treating patients and unable to answer the phone. However, if you prefer to call, leave a message and I will get back to you ASAP!
YOU MUST SCHEDULE AN EVALUATION FIRST - BEFORE ANY TREATMENT SESSIONS CAN BE SCHEDULED.
Payment at Embody PT
Embody PT is an out-of-network provider for most commercial insurances with the exception of Blue Cross Blue Shield of VT. This means we do not accept insurance and are not able to bill insurances directly except if you have a Blue Cross Blue Shield plan. If you don't have BCBS, you will be provided a super bill with proper codes which you can submit to your insurance plan to attempt partial or full reimbursement, depending on your plan. Call your insurance provider to see what your out-of-network benefit for physical therapy entails. Embody PT is an in-network provider for BCBS of Vermont and can bill directly for that insurance only. If you have another BCBS plan, you may still be eligible for in-network coverage. Embody PT is not a Medicare provider clinic and is unable to provide super bills for Medicare patients. For all patients, payments are due at time of evaluation and treatment - for those with BCBS of Vermont, this includes co-pays, co-insurance and any costs before reaching the deductible.
Cancellation at Embody PT
A credit card will need to be provided to schedule your initial evaluation. This card will be charged a $1 holding fee until the evaluation is completed or in the case of late cancellation or no show for the appointment.
If you need to cancel any of your appointments, please provide a minimum of 24 hours notice. If you cancel after 24 hours or no show for the appointment, a $50 cancellation fee will be charged to your credit card.
